The Moscow Urban Forum will show what modern emergency assistance should look like – Rossiyskaya Gazeta

Date: September 30, 2023 Time: 03:20:33

Yuri Vladimirovich! Is your flagship almost ready?

Yuri Barinov: Heat, electricity and water supply. The heavy equipment is in place. Anesthesiology and surgery consoles are already hanging in the operating rooms. In short, the degree of preparation to receive patients is the highest possible.

You saw the flagships in Sklifa and the hospital. Is Veresaeva already at work?

Yuri Barinov: Of course, and more than once. It is important to understand what characteristics we can have in the acceptance process. Now, for example, we are seeing how the new centers work without a paper medical record, how they have changed to an electronic format.

How is your center different from the open ones?

Yuri Barinov: Each flagship has its own architectural appearance. Ours seems to me the most beautiful, it has seven floors, with a helipad. In terms of capacity, it is the same as in Sklifa, designed to receive about 200 emergency patients per day. It will feature a modern hybrid operating room, where several teams of surgeons will be able to work on the same surgical table at the same time, using different technologies and capabilities. In critical situations, this will save time. The entire fifth floor is devoted to the operational block.

It only takes a minute or two for medics to start rescuing a patient admitted through the red zone.

But are the basic principles of all centers the same?

Yuri Barinov: Yes. Each has a heated vestibule; four ambulances can enter immediately. At the entrance you will be greeted by an experienced doctor and nurse. They decide where exactly each patient should be referred. Lung – to the green zone, where it is examined in detail and the necessary assistance is provided. Moderately severe – in the yellow zone, patients with strokes, heart attacks, victims of traffic accidents, whose lives are in danger, in the red zone, if necessary, in the anti-shock room. Transport takes literally a minute or two, and the patient will be rescued immediately. To do this, there is all the necessary equipment. All zones work on the principle: the doctor to the patient – specialists examine him already in bed, using mobile equipment. Here they will draw blood and do other necessary tests. Diagnosis and maximum assistance are provided from day one. For this, our flagship has two CT scanners, an MRI plus two fluoroscopy rooms and an entire endoscopy department with four endoscopy operating rooms. Thanks to this, for example, we can remove a stone from the bile duct immediately after entering the flagship center. Under normal conditions, such a patient is first hospitalized and examined, and only the next day they can operate on him.

With the launch of the flagship, we will also be able to receive much more patients with intestinal obstruction, with pancreatic necrosis – we have the best results in the treatment of such diseases in the city, and, as a rule, an ambulance takes patients to Botkinskaya. Having received emergency assistance, our patients will be able to receive additional treatment in 10 specialized centers, where a whole constellation of medical luminaries works at the Botkin Hospital.

Tell us about your flagship design, which will be shown at the Moscow Urban Forum.

Yuri Barinov: Our entire center is made in miniature 60×60 centimeters in size. Each floor can be taken out and examined in detail. The “Triage” zone, where intelligent algorithms help distribute the flow of patients into zones, according to the urgency of the aid. Comfort zone for the patient’s companions. They do not need to go to the doctors to find out the status of a relative. All information is transmitted on the screen, but encrypted, it is understandable only to relatives. resuscitation department. A diagnostic department for patients who need to be monitored by doctors for at least one day before making an accurate diagnosis. Rooms for one or two beds, with shower and toilet. A complete examination may show that someone does not need hospitalization at all, and then the person will be discharged for outpatient treatment. Well, and those who still need hospital treatment, of course, we will treat them. The entire modern operating block is specific to Botkinskaya, since we work on a large number of surgical profiles and this, of course, will leave its mark on the activities of the new center. The helipad on the roof, of course, did not appear by chance. By air, without traffic jams, they are most often delivered after an accident with combined injuries, damage to both the brain and the chest. The sooner the patient arrives at our center and the sooner the doctors start rescuing, the better their chances of survival.

Equipping flagship centers with the best equipment is important for another reason. With its help, a disease can be detected that the patient may not be aware of. Therefore, we will give you the opportunity to have an operation and start treatment in a planned and non-urgent manner, when the question of life and death already arises.

Yuri Vladimirovich, why should ordinary Muscovites, not doctors, consider the design of the new flagship? To delve into your device?

Yuri Barinov: To find out what tremendous opportunities a simple compulsory health insurance policy now opens up for them. And if problems arise, be sure that the flagship center will definitely help you.

Help “RG”

The introduction of a new standard of emergency care for the health of the capital is one of the key tasks. By the end of 2013, six flagship centers will open in the city. They are located so that the closest one is anywhere in the city. The first center opened in January at the hospital that bears his name. Veresaev. It has already received more than 20 thousand patients. About 40% of the townspeople were brought there with a heart attack and stroke, about a third – with appendicitis, cholecystitis, pancreatitis and other acute pathologies. More than 15% – with injuries and poisoning. Approximately one in five admitted doctors took the red route and immediately began fighting for his life, 37% were hospitalized for the yellow route, the rest for the green zone.
